High-Risk Pregnancy Tutorial

 

Unit 1: Key Concepts

This unit provides information about five key concepts in the care of a client experiencing a high-risk pregnancy. These concepts form the foundation for an approach to care for a client with a high-risk pregnancy, and include:

  • Timing
  • Interactivity of the maternal-fetal unit
  • Psychosocial implications
  • Principles of self care
  • Social support

Unit 2: Assessment and Management for Pre-Existing Conditions

This unit discusses some common pre-existing diseases and their impact on the pregnant woman. The unit specifically focuses on

  • Cardiac disease
  • Dabetes mellitus
  • Hypertension
  • Perinatal infections. Specific infections include those classified as
    • TORCH infections
    • Bacterial vaginosis
    • Candidiasis
    • Chlamydia
    • Gonorrhea
    • Hepatitis
    • Human papillomavirus
    • Syphilis trichomoniasis
    • Pelvic inflammatory disease

Unit 3: Assessment and Management of Pregnancy-Induced Conditions

The pregnancy-induced conditions covered in Unit 3 are grouped into three categories:

  1. Bleeding disorders
  2. Preeclampsia/eclampsia
  3. Preterm labor. 

Specific conditions covered under bleeding disorders include

  • Ectopic pregnancy
  • Gestational trophoblastic disease
  • Spontaneous abortion
  • Vaginal spotting
  • Abruption placenta
  • Placenta previa
  • Ruptured uterus
  • Disseminated intravascular coagulation and Rh sensitization are discussed in this unit where appropriate. 

In the section on preeclampsia, both mild and severe preeclampsia are discussed as well as eclampsia and HELLP syndrome. 

The effects of preeclampsia on both the mother and baby are discussed.

When discussing the effects on the baby, fetal monitoring and various fetal heart rate patterns are reviewed.

Finally, conditions related to preterm labor including premature cervical dilation, premature rupture of the membranes, and premature uterine contractions are covered.

 

Unit 4: Fetal Well-Being and Maturity

Many of the conditions which place a pregnant client in a high-risk status can affect fetal well-being and maturity. This section covers tests to monitor the fetus and is divided into two sections:

  1. Fetal well-being
  2. Fetal maturity

The section on fetal well-being covers

  • The diagnostic tests of amniocentesis
  • Biophysical profile
  • Chronic villus sampling
  • Contraction stress test
  • Fetal movement
  • Maternal serum alpha-fetoprotein
  • Non-stress test
  • Utrasound

The diagnostic procedures to monitor fetal maturity include

  • Amniocentesis
  • Fundal height measurement
  • Ultrasound.

Unit 5: Self Test

This unit consists of a test bank of 20 items. The test bank questions were designed to address all levels of difficulty. 

Knowledge and comprehension questions are used to ensure the student has mastered the necessary foundation. 

Application and analysis questions are used to test the student’s ability to apply the information to client situations. 

Immediately after answering a question, the student is given feedback. This feedback is helpful in correcting any misunderstandings the student may have. 

After taking the test, the student may print out the results. Feedback relative to the correct answer is provided. 

The questions contained in the self test are included in Chapter 6 of the manual. You may use these questions when preparing your own classroom tests.  Because this is a “self test”, students may exit the test bank at any time.
